Hair fall and dendruff

Its been almost a year with this issue. My hairs have become very less dense and i am struggling with dendruff. Please provide with a shampoo and some remedy for this issue. I would be greatful to you.

Do you have family history of hair fall on your maternal or paternal side. Have you taken any treatment for it. Can you please send me your pictures for better diagnosis and treatment. Hair fall can be stopped but need pictures. Brand names can't be written in open forum.
