You may be suffering from androgenic alopecia which is also called as pattern baldness. The hairs start reducing in density at the crown (top) and also the hairline starts refeeding from the front. It is androgenetic alopecia. Central patch can be treated with medicines with excellent results.
For hair line, you will need transplant if you wish to have your original hairline. If you are okay with this hairline, i would advice you to go for medical management only.
