General checkup
Hi Doctor, actually i want to know if this foreskin issue can be solved at physician level, as my foreskin retracted during growing age and stayed behind glans i thought this as normal growing process causing permanent exposure of glans since then. There is not any infection visible in normal flaccid case as seen in attached image. But now i got to know it should retract and come back again to cover glans. So i thought to check by sliding my foreskin back to get outermost foreskin tip location i found there is some pinkness and adhesion of outermost foreskin tip with shaft, looking like it merged with shaft by a tissue see attached image. Is due to this reason my glans not covering by foreskin in flaccid case also ?It will be helpful if you let me know this disease name. Also if it can diagnose by you let me know or else let me know which specialist doctor should i proceed for urologist, surgeon or dermatologist ?

My suggestion is  to consult a Urologist. This is an issue which has been long standing and the signs of chronicity( adhesions) are already there.

Based on your description, this condition is likely a combination of Phimosis (tight foreskin) and Preputial Adhesions, where the inner lining of the foreskin has fused with the glans or shaft. Since your glans remains permanently exposed and the skin is not gliding back to cover it, it could also indicate a chronic state of Paraphimosis. The 'pinkness' and 'merged tissue' you noticed are typical signs of long-term adhesions that require professional intervention."
Next Steps
I strongly recommend you consult a Urologist for a physical examination. While minor adhesions can sometimes be treated with steroid ointments and manual stretching under medical supervision, chronic cases might require a small surgical procedure like Circumcision or Preputioplasty to provide a permanent solution and maintain proper hygiene."
Health Tips
Do not try to forcefully pull or slide the skin back yourself, as this can cause micro-tears, bleeding, and further scarring, which will make the condition worse. Avoid applying any random over-the-counter creams until a Urologist confirms the diagnosis. Maintaining local hygiene is important, but gentle care is the key until you see a specialist."

Thank you for your detailed explanation. Based on your history, this does not appear to be an infection. The condition you are describing is most likely due to either a normal variation where the foreskin remains retracted, or mild adhesions / tethering of the foreskin to the shaft skin. In some individuals, the foreskin does not fully cover the glans in the flaccid state, and this can be normal. The pinkness you are noticing is likely due to exposure and friction rather than any disease. However, the adhesion you mentioned may need a physical examination to confirm whether it is: simple adhesion, short frenulum , any structural tightness. This condition is usually not serious and can be treated if required. I would advise you to consult a urologist, as they are the most appropriate specialist for this concern. Treatment, if needed, can range from simple measures to minor procedures. Avoid forcefully pulling the foreskin, as it may cause injury. If you develop pain, difficulty in erection, or signs of infection (redness, discharge, swelling), seek medical attention earlier. Overall, this is a manageable condition and not something to worry about. Thank you.
