Flaky scalp and greying of hair

I an 24 years old i have had dandruff and grey hair for a long time but in the recent days it has increased a lot leading to patches all over my scalp. i keep scratching it all the time as it itches a lot and also there is an increase in the graying of hair . what do i do ?
