Hello. A fever that does not subside with Dolo, combined with significant tonsil pain, usually indicates that the infection is more than a simple cold and may be bacterial in nature. Immediate Self-Care: Betadine Gargles: Use Povidone-Iodine (Betadine) germicidal gargle diluted with warm water every 4–6 hours. This is much more effective than plain salt water for active tonsil infections. Hydration: Drink plenty of warm fluids. Avoid cold or citrus juices, which can irritate the inflamed tonsils. Rest: Ensure complete physical rest to help your immune system fight the infection. Medical Management (Standard Indian Brands): For Pain & Fever: Switch from plain Dolo to Tab. Zerodol-SP (Aceclofenac + Paracetamol + Serratiopeptidase) twice daily after food. The 'Serratiopeptidase' helps specifically in reducing the swelling of the tonsils. For Throat Infection: Based on your description, a course of antibiotics like Tab. Azithral 500mg (once daily for 3 days) or Tab. Augmentin 625mg (twice daily for 5 days) is often required. Note: Please consult a doctor in person or via video for a formal prescription for antibiotics. Anti-allergic: If there is a lot of phlegm or a runny nose, Tab. Allegra 120mg once daily can help. When to seek urgent medical help: If you find it difficult to swallow even liquids or saliva. If you have difficulty breathing or a muffled/changed voice. If you cannot open your mouth fully.

Since you have fever for two days and is not responding to paracetamol, it may be a strep throat or a viral infection that requires evaluation
Next Steps
Drink plenty of warm fluids and gargle with warm salt water every few hours

Continue paracetamol, do salt water gargles and steam. Most throat infections settle in 3 days. If fever or pain doesn’t reduce or worsens, you may need antibiotics after examination U may need physical or online consultation
