If fever is still present after antibiotics and 4days of pcm. You should get him tested for malaria and a blood culture should be done. Also he should be checked physically by a physician if there are any other signs.

Please get Fever profile tests done and consult with a General Physician in your area for a physical examination and further management. He should have got the fever profile done many days ago. Self medication with Antibiotics is dangerous.

Dear Patient, It’s concerning that your brother has been experiencing a fever for 8 days despite taking antibiotics and paracetamol, with no other significant symptoms and negative results for dengue, typhoid, and chikungunya. Persistent fever can have many potential causes, and since common infections have been ruled out, here are the next steps to consider: Next Steps: Complete Blood Count (CBC): Recheck CBC to monitor any changes in white blood cell count or indicators of infection or inflammation. Blood Culture: If bacterial infections are suspected but not responding to antibiotics, a blood culture can help identify any specific bacteria that may not have been covered by the initial antibiotics. Malaria Test: Although it’s less common in some regions, malaria should be ruled out, especially if he has been in areas where it’s prevalent. Viral Fever: Sometimes, viral infections can cause fever lasting 1-2 weeks without other obvious symptoms. In this case, supportive care with hydration, rest, and paracetamol for fever management is important. Consult a Physician: It’s essential to consult a doctor for further evaluation. They may suggest imaging (like a chest X-ray) or other tests depending on his examination.
Next Steps
If the fever continues, it is important to follow up with a healthcare provider to determine if further testing or a different course of treatment is needed. Please let me know if you need more guidance!

It's concerning that your brother has been experiencing fever for the last eight days. Given that he has already taken antibiotics and paracetamol, and considering the absence of other symptoms apart from headache and body pain, it may be necessary to explore further evaluations. The intermittent fever pattern could indicate several underlying conditions, such as viral infections, reactivated infections, or other systemic issues. Since dengue and typhoid tests have come back negative, it would be wise to consider further tests, including blood counts, liver function tests, and possibly a chest X-ray, depending on his clinical status. I strongly recommend consulting a healthcare professional for a thorough evaluation and appropriate treatment. You can reach out to me for better evaluation and treatment options by contacting me on the number eight zero zero nine eight two one three three two.

I'm sorry to hear about your brother’s condition. It’s good that the tests for dengue, typhoid, and chikungunya are negative, but since the fever persists after antibiotics were discontinued, it would be wise to consider the following steps: 1. **Reevaluation by a Doctor**: Since the fever is recurring, I would recommend a thorough reassessment by a physician. The fact that the fever subsides with paracetamol but returns suggests there may be an ongoing infection or another underlying condition that requires attention. 2. **Blood Tests**: If not already done, a **complete blood count (CBC)** and **CRP (C-reactive protein)** could help assess for any lingering infection or inflammation. Other tests like **LFTs (liver function tests)** and **KFTs (kidney function tests)** may also be useful if the fever persists. 3. **Consider Malaria**: Malaria can present with periodic fevers and should be ruled out, especially in endemic areas. If malaria testing hasn’t been done yet, it may be worth considering. 4. **Continue Monitoring**: It’s crucial to keep monitoring his temperature, and hydration is important. Sometimes viral infections can cause lingering fevers, but if there’s no improvement after a few more days, further investigations may be needed. 5. **Hydration and Rest**: Ensure he stays well-hydrated and gets adequate rest, as this will support his recovery. Since he's in Haldwani, Uttarakhand, a visit to a local healthcare facility to reassess his situation would be the best course of action if symptoms persist. Please keep track of any new symptoms that might arise and avoid overuse of paracetamol. If the fever continues beyond a few more days, he may need imaging studies or more specific tests based on clinical findings.
