Why is fever coming and going? This is normal in viral infections. Most viral fevers (flu, viral cold, dengue-like viruses, etc.) show a pattern where: • Fever rises → medicine reduces it • After 6–8 hours, fever comes again • This continues for 2–4 days It does not mean the medicine is not working. It is simply the natural cycle of a viral fever. ⸻ Why severe body pain, especially leg pain? This is also very common with viral fever. Reasons: 1. Viral myalgia • Virus causes inflammation in muscles • Leads to severe body pain, back pain, and especially calf/leg pain 2. Dehydration due to fever • Causes muscle cramps and heaviness in legs 3. Weakness after fever drops • Body pain is sometimes worse when temperature comes down This is seen in: • Common viral fever • Influenza • Viral cold • Dengue (pain is intense, but fever usually high) ⸻ How long do tablets take to work? • Fever medicines (paracetamol) work in 30–60 minutes, but only last 6–8 hours • Body pain improves more slowly • Most viral infections need 3–5 days for full recovery So it is normal that: • Fever reduces but comes back • Body pain remains even when fever is low ⸻ What to do now 1. Continue prescribed medicines exactly as doctor advised 2. Give plenty of fluids: ORS, coconut water, soups 3. If fever > 101°F, give paracetamol 650 mg every 6–8 hrs 4. Warm water bath or warm compress for leg pain 5. Gentle stretching helps relieve calf pain
Next Steps
When to seek review Go to doctor if: • Fever lasts more than 3 days • Very severe leg pain • Vomiting • Severe headache • Rashes • No urine for 6–8 hours These may suggest dengue or dehydration. ⸻ Most likely diagnosis Simple viral fever with viral myalgia → very common, usually improves in 3–4 days with medicines.

Hello, I am Dr Romain Rajan, General Physician. Your case has been allotted to me. I understand your concern. Fever that comes and goes along with severe body pain, especially leg pain, is very common during viral infections. Many patients worry when fever drops but body pain continues, but this is a normal phase of recovery. Advices 1 Keep him well hydrated. Give warm water, soups or ORS as fever increases dehydration and worsens body pain. 2 Allow him to rest properly. Overexertion during fever makes the body pain more intense. 3 Use warm compress for leg pain and mild stretching only after the pain reduces. What to Monitor 1 Fever rising above one hundred and two degrees repeatedly. 2 Extreme weakness or dizziness. 3 Severe headache or vomiting. Management Viral fever often causes • Fluctuating fever • Severe muscle pain • Leg and joint pains • Tiredness Even after the fever reduces with tablets, body pain can continue for two to three days because the muscles remain inflamed. This does not mean the medicine is not working. If the doctor has given an antibiotic or antiviral along with paracetamol, it can take at least forty eight to seventy two hours for full relief. For leg pain, a safe anti inflammatory tablet or muscle relaxant may be needed depending on his health and sugar level. I can guide you with the exact dose after knowing the medicines he already took. When to Consult a Doctor Immediately 1 Fever not reducing even with medicines 2 Severe leg pain that prevents walking 3 Breathlessness or chest discomfort 4 Continuous vomiting Every patient’s situation is unique. For accurate diagnosis and lasting relief, it is important to discuss his symptoms, medicines taken and underlying health conditions like diabetes in detail. You can reach out to me directly for this case and also connect with me in future for any medical consultation. I am available on Practo or on WhatsApp at 85271646 seven seven for a one on one consultation where you will receive 1 A personalised treatment plan 2 Safe medicine adjustments if required 3 Quick follow ups to track his recovery Take care, wishing your husband good health. 1. Why is fever coming and going? This is very common in viral infections like flu, viral fever, dengue, etc. Reasons: Viral fever fluctuates — it rises when the infection activity increases and drops after medicines (paracetamol). Paracetamol effect lasts only 6–8 hours, so fever can return when its effect wears off. The immune system also naturally causes fever spikes. As long as the fever is reducing with medicines, it is usually okay. 2. Why severe body pain even when fever comes down? Body pain (especially leg pain, muscle aches) is also common in viral infections. Possible reasons: a) Viral infection muscle inflammation Virus releases chemicals → causes myalgia, fatigue, leg pain. b) Dehydration Fever → sweating → loss of water → muscle cramps and pain. c) Some viral fevers (like dengue) cause very intense body pain If pain is too severe, check for: persistent high fever headache behind eyes vomiting rash weakness (If yes → get medical test) d) Slow action of medicines If he just started treatment today, it usually takes 24–48 hours to feel real improvement. How long do tablets take to work? Fever medicines work within 30–45 minutes, but only temporarily. Full recovery from a viral fever usually takes: 3–5 days (sometimes up to 7 days). What you can do at home ✔ Give plenty of fluids: ORS, water, coconut water ✔ Continue prescribed medicines ✔ Give Paracetamol every 6–8 hours (if doctor advised) ✔ Lukewarm sponge if fever high ✔ Light diet, rest Take him to hospital if: ❗ Fever continues beyond 3 days ❗ Severe leg pain not improving ❗ Vomiting, dizziness ❗ Breathlessness ❗ Not able to drink water ❗ Decreased urine
