Hi — I’ve gone through your detailed description carefully. You’re clearly going through a bacterial lower respiratory infection (likely acute bronchitis or early pneumonia) that hasn’t fully responded to your current medicines. Let me explain step by step so you can recover quickly and safely What’s happening The high ESR (64) and green sputum both point toward bacterial infection rather than viral. Since you’ve already completed 3 days of Augmentin Duo 625 mg, the infection may be partially resistant or deeper in the bronchi/lungs. The persistence of high fever (102.6°F) even after 3 days suggests we need to reassess and slightly upgrade the treatment plan. Next step in treatment 1. Change/upgrade antibiotic: Switch to Levofloxacin 500 mg once daily after food for 5 days (if no history of allergy) — this covers the most common resistant strains causing bronchitis and atypical pneumonia. → Stop Augmentin if you start this. 2. Continue symptomatic medicines: Tab Montair (montelukast) – once at night. Syrup Rapitus LS – 10 ml thrice daily after meals. Sporlac DS – continue twice daily for gut protection. 3. Add anti-inflammatory & antipyretic: Tab Paracetamol 650 mg every 6 hours if fever >100°F. Tab Acebrophylline 100 mg twice daily (to reduce mucus & inflammation). 4. Supportive measures: Steam inhalation 3 times/day. Drink plenty of fluids, especially warm soups and water. Avoid dust, cold air, and talking excessively — rest your lungs. Tests you should do now If possible, get: Chest X-ray (PA view) → to rule out pneumonia. CBC with differential count → to check WBC pattern. CRP → to assess severity of inflammation. When to seek urgent help Persistent fever >101°F after 48 more hours Breathlessness or chest heaviness Rust-colored or blood-streaked sputum These would need evaluation for pneumonia or secondary infection. Reassurance You don’t need to worry — this is a treatable condition, and with the right antibiotic adjustment and proper follow-up, you’ll recover completely within 5–7 days. Once fever settles, the cough will gradually improve over the next week. Since you’re eager to get back to work, I can tailor your antibiotic and supportive therapy exactly as per your latest reports and symptoms. You can message me directly on WhatsApp at nine three two six zero two zero five three six — send a picture of your blood reports and medicines you’re currently taking. I’ll review them and guide you with a fast-track recovery plan so you can safely resume work within a few days.

Is there any history of Weight loss or loss of appetite in recent? This might be Pulmonary and Extrapulmonary tuberculosis. Because given symptoms with raised ESR. Evening rise of temperature. This all points towards tuberculosis. You need to be investigated further. Get a Chest X-ray PA view done and then book a consult. We can plan your further management protocol.
Next Steps
Get Chest X-ray PA view done
