Esophagus reflux hypersensitivity
I had symptoms of severe acid reflux and not getting sleep due to reflux. I am diagnosed with esophageal reflux hypersensitivity as per 24h pH impedance test conducted at AIG hospitals, Hyd. Also, been detected for GRADE 1 FATTY LIVER in the USG abdomen test. I had low levels of Vit D3 and B12 for which I took injections.   Other tests are normal. Doctor has prescribed the following medication for 3 MONTHS: TAB CINTOSHEL PR  (1 in morning before food) TAB NEXITO PLUS ( 1 at night after food) CAP GLUHEP ACTIVE 500mg ( 1 in morning and 1 at night after food. I have following queries:   1. Do the NEXITO PLUS is habit forming ? Do I get addicted after finishing 3 months course since it is prescribed normally for anxiety and depression? Is NEXITO PLUS really for ACID REFLUX? 2. I am now getting acidity in the back between shoulder blades. Does tab CINTOSHEL cures this issue? 3. Can I take other supplements like SHELCAL 500 for calcium , NEUROBIN PLUS for B12, UPRISE for vit D3?

I have reviewed your reports and treatment plan. Let me address your queries one by one: 1. About Nexito Plus (Escitalopram + Clonazepam): • This medicine is mainly prescribed for anxiety, depression, and sleep disturbance. In reflux hypersensitivity, even when acid reflux is not severe, the nerves of the esophagus become over-sensitive and amplify the symptoms. • In such cases, low-dose anti-anxiety/neuromodulator medicines like Nexito Plus are used to reduce esophageal hypersensitivity and improve sleep. • Addiction risk: Escitalopram is not habit-forming. The clonazepam part, if used long term or in high doses, can cause dependence, but in a short 3-month supervised course the risk is minimal. You should not stop it suddenly on your own—your doctor will guide tapering if needed. • So, yes, it is prescribed to help with your reflux-related symptoms by calming hypersensitivity, not just for depression. 2. About Cintoshel PR (Itopride + Rabeprazole): • This combination has a prokinetic (improves stomach emptying, reduces reflux) and a PPI (reduces acid production). • It helps in symptoms of heartburn, reflux, and upper back/chest discomfort due to acid. • However, in reflux hypersensitivity, medicines reduce symptoms but may not fully cure—lifestyle modification is equally important (avoid late meals, elevate head while sleeping, avoid spicy/fatty foods, no lying down soon after food). 3. About Gluhep Active (Glutathione + other liver support): • This is given for fatty liver Grade I. Along with it, weight control, regular exercise, and diet modification are most effective. 4. Taking other supplements: • Shelcal 500 (Calcium): Safe to take. • Neurobion Plus (B-complex with B12): Safe, especially if your B12 was low. • Uprise (Vitamin D3): Safe and recommended if your D3 was low. • Just keep a 2–3 hour gap between these supplements and your reflux medicines (like Cintoshel PR), as absorption may be affected.
