Hello, I am Dr. Romain Rajan, General Physician (Ex Govt Medical Officer at RML Hospital, New Delhi), and your case has been allotted to me. Thank you for sharing your details. I understand your concern. Based on your history of right upper abdominal dull pain, fatigue, elevated SGPT (77), and known fatty liver, the most likely cause is fatty liver related inflammation (NAFLD). ⸻ What this means • Mild elevation of SGPT indicates liver inflammation • Fatty liver can cause dull pain under right ribs and fatigue • Your bilirubin is near normal, which is reassuring ⸻ What you should do now • Strict lifestyle modification is the main treatment • Avoid oily, fried, processed foods and alcohol • Start regular exercise at least 30 to 45 minutes daily • Aim for gradual weight reduction if overweight ⸻ Investigations to consider • Repeat LFT after 6 to 8 weeks • Ultrasound whole abdomen for current status • Lipid profile and blood sugar monitoring ⸻ Treatment approach • No specific medicine required in most cases • Focus is on diet, exercise, and liver recovery • Antioxidant supplements may be added if needed ⸻ When to consult a doctor immediately • If pain becomes severe • If jaundice, vomiting, or loss of appetite develops • If LFT continues to rise ⸻ Important reassurance Fatty liver is very common and completely reversible in early stages with proper lifestyle correction. ⸻ For a structured diet plan and liver recovery guidance, you can reach out to me directly on WhatsApp at 85271646 seven seven. Wishing you good health. Dr. Romain Rajan General Physician

Thanks for sending across your query. Plz repeat the USG whole abdomen again to know the cause of pain. Try to avoid all the oily fried and spicy food in your diet. Avoid all the processed food.

• Your symptoms + reports point towards liver-related cause: – Recurrent right upper abdominal pain – Fatigue – SGPT 77 (elevated) – Past history of fatty liver
Next Steps
• Confirm current liver status: – Repeat LFT after 4–6 weeks – Ultrasound abdomen (to reassess fatty liver grade) • Additional tests (if persistent): – Viral markers (Hepatitis B, C) – Lipid profile (already done) • Management (main treatment = lifestyle): – Weight reduction (target 5–10%) – Regular exercise (at least 30–40 min/day)
Health Tips
• Strictly avoid: – Alcohol (if any) – Sugary drinks, refined carbs • Diet: – High protein + high fiber – Reduce oily/junk food • Coffee (in moderation) can actually help fatty liver • Sleep and stress control also improve liver metabolism

Fatty liver can be due to obesity, high insulin resistance. Diabetes, excess alcohol consumption. It can be corrected by fat free diet, regular physical exercise. If severe, statins are given to reduce it.

Your reports suggest liver inflammation. Could be because of the fatty liver which can cause the pain in right upper quadrant. Also hba1c 5.8 is in prediabetic range .
Next Steps
Try including exercise . Once again get your LFTs checked if not recent and get a diabetes screening test with FBS,PPBS also.
