Hi
After having dull pain under right abdomen, we did lft where total bilirubin was 1.2 and sgpt was slightly higher 77.
Post diet changes over a month, sgpt came down to 61 plus improvement in discomfort.
Now Total bilirubin increased to 1..32 (indirect value now is 0.89 from 0.78).
All other markers like ggtp etc have also improved.
Why did bilirubin increase post diet changes?

The overall trend sounds reassuring since your SGPT and discomfort have improved 👍
Small fluctuations in bilirubin can happen for several reasons and are not always dangerous, especially when other liver markers are improving. A proper review of your reports and lifestyle factors would help understand it better. You can book a consultation, I’ll guide you step by step 😊

"The slight increase in your total bilirubin (1.32 mg/dL) despite an improvement in SGPT (from 77 to 61) and GGTP is actually a common clinical observation during the recovery phase of fatty liver or mild liver inflammation. Since the increase is primarily in the Indirect Bilirubin fraction (0.89), it is often not a sign of liver damage worsening, but rather a temporary 'lag' in how the liver conjugates (processes) bilirubin. This can sometimes happen during significant diet changes, weight loss, or even mild dehydration. If you have no yellowing of the eyes or dark urine, this small fluctuation is generally not a cause for alarm
Next Steps
Hydration is Vital: "Ensure you are drinking at least 3 liters of water daily. Dehydration can cause a temporary rise in indirect bilirubin levels."
Avoid 'Crash' Dieting: "Ensure your diet changes are sustainable and balanced. Very low-calorie diets or sudden fasting can sometimes cause bilirubin to fluctuate temporarily."
Limit Toxins: "Continue to strictly avoid alcohol and processed sugars, as these put direct pressure on the liver's ability to conjugate bilirubin."
Caution: "Do NOT worry about minor fluctuations in decimal points (like 1.2 to 1.3). Focus on the 'big picture'—your SGPT is down by 16 points, which is a major victory for your liver health."
Red Flags: "Seek immediate medical advice if you notice yellowing of the skin/eyes (jaundice), clay-colored stools, or if the right-sided pain becomes sharp and severe
