Difficulty ejaculating during sex
I have difficulty ejaculating during sex. I can maintain an erection and continue intercourse for a long time, but I cannot reach ejaculation or orgasm. This has been happening consistently and is affecting my sex life. What could be causing this, and what tests or treatments would you recommend?

Difficulty reaching ejaculation despite maintaining erection is a condition called delayed ejaculation (DE), and it is more common than most people realise. Common causes include psychological factors (performance anxiety, relationship stress), excessive masturbation with a very specific grip or stimulation type, certain medications (especially antidepressants, SSRIs, antihypertensives), or less commonly hormonal issues like low testosterone or thyroid dysfunction. It is important to identify the underlying cause before treatment.
Next Steps
Please consult me directly on Practo for a detailed evaluation — I can take a confidential history and guide you on the right investigations and approach.

Your ability to maintain a satisfactory erection suggests that erectile dysfunction is unlikely. The problem appears more consistent with delayed ejaculation or difficulty achieving orgasm. Common causes include psychological factors (performance anxiety, relationship stress) certain medications (especially antidepressants), hormonal abnormalities (low testosterone, high prolactin), diabetes-related nerve dysfunction, and less commonly neurological conditions. I would recommend evaluation by a psychiatrist specializing in sexual medicine, an andrologist, or a urologist.
Next Steps
Basic tests include fasting blood sugar/HbA1c, thyroid profile, morning testosterone, and prolactin levels. Treatment depends on the underlying cause and may involve sex therapy, medication review, or correction of hormonal abnormalities.

Delayed ejaculation can occasionally be linked to underlying chemical or structural factors. These include a decrease in overall thyroid or testosterone hormone parameters, mild nerve desensitization in the pelvic area, or a condition called retrograde ejaculation, where semen travels backward into the bladder instead of exiting forward.
Next Steps
To structurally review your complete sexual health history, analyze your upcoming hormonal profiles side-by-side, and map out a safe, customized treatment routine to restore your peace of mind and sex life, please book a full consultation session via my Practo profile."
