Dandruff And Itch - I Have Dandruff For 6 Months Now And My Hair

I have dandruff for 6 months now and my hair is getting dry. On the back of my neck beliw my head there is always an itching feel and i keep scratching. My skin is shedding there. What can i do to stop all this dandruff and itch?
