Sir, I had malaria (P. vivax) in September. After completing the treatment, I was tested again, and the results showed that the malaria was negative and my RBC count was normal.
However, I continued to experience significant dizziness and a sensation of internal body vibrations, similar to shivering. Around November, I also developed a persistent cough.
I became concerned and consulted a doctor. A chest X-ray was done, and it came back normal. The doctor suspected sinusitis and prescribed medications including levocetirizine, antibiotics, and a nasal spray. I took them for about 15 days, and the cough improved somewhat but did not completely go away, so I eventually stopped the medications.
Now it has been about 7–8 months, and I still produce mucus/phlegm regularly. I spit out mucus around 15–30 times a day. I often feel that the mucus is draining from my nose into my throat and then into my mouth.
Yesterday, I noticed blood in my sputum/phlegm, including a small clot-like structure

Persistent mucus production for 7-8 months with post-nasal drip after a chest X-ray came back normal suggests chronic upper airway cough syndrome (post-nasal drip) or chronic sinusitis as the most likely cause. However, a productive cough lasting this long also warrants ruling out tuberculosis â TB can occur in people whose immunity was stressed by an illness like malaria, and a normal chest X-ray does not fully exclude it.
Next Steps
Please see a pulmonologist or ENT specialist. Get a sputum test for AFB/TB culture and a sinus CT scan done. Do not leave a 7-8 month cough undiagnosed. Please consult me directly on Practo for a detailed evaluation.
The patient needs to understand that while his symptoms might still be caused by severe sinus drainage, spitting up blood clots is a symptom that cannot be self-managed or ignored. He should stop looking for over-the-counter remedies and present to a specialist clinic or hospital emergency department for an HRCT chest and sputum analysis to safely isolate and treat the source of the bleeding.
