Hello
If the tooth is causing headache, mostly the cavity is very deep and involving the pulp or the nerve of tooth.
You will have to get it properly checked by a dentist. You can always tell your concerns that you dont want to get the tooth removed. And the dentist will definitely help you with alternative treatment options such as filling or RCT.

If you are having severe pain this indicates it might have reached nerves. U should get xray done . U dont have to get rid of it. If tooth is in restorable condition we can save tooth by doing root canal treatment followed by crown.
Next Steps
get xray done. so that proper treatment can be suggested
Severe pain indicated that decay has reached nerve tissue of your tooth and there is infection. Simple filling won't solve your problem.Get xray done and either go for RCT(if it's possible) or go for extraction. Is there any particular reason that you dnt want extraction now? Share xray of you have.

Health Tips
As your diabetic please get this particular tooth treatment at earliest.
U should not go for filling rather first share ur current x ray. If the condition is good I think it can be saved via RCT or else v have to for extraction
