From the picture I cldn't come to a conclusion yet, Watch the eati g habit and gestures of the child and figure out whether he/she has tooth pain or not. Then pay a visit to a pedodontist (since the child is 20 month a specialist is needed)
Next Steps
1.he/she may perform a full mouth scaling (cleaning of debris form the tooth) 2.If caries is present kindly without any doubts go for a restoration (since the child is 20 month they cant obey or follow regulations and routines it may further develop into a painful lesion) 3.Followed by an Topical fluoride application.
Health Tips
make a habit of rinsing his/her mouth after a meal/chocolate/juicec/beverages,etc.

Hello If you are observing cavities in your child mouth visit the dentist and get it restored. After restoration the cavity will not progress. yes it is a sign of early decay or may be so extrinsic deposit on tooth surface that can only be confirmed by dentist
Next Steps
Try to avoid sticky food and processed food for your kid
Health Tips
If not introduced yet please start brushing your childs teeth with flouride free tooth paste. try not to give anything to eat after brushing at night.
