Burning Sensation and Oral Mucosa
Hello Doctor, I have been facing a burning sensation inside my mouth for the last 1 year. It is worse in the evening. There are white line/patch-like changes on the inner lips and oral mucosa. The area feels very sensitive and sometimes sticky. Spicy food increases irritation. I consulted many doctors, including an oral medicine specialist, but was told it is nothing serious and may heal on its own. However, the problem is still persistent. I tried steroid creams and other topical treatments but got no relief. B-complex did not help. After taking antioxidants like lycopene, burning increased. Earlier I had Vitamin D deficiency, but even after correcting it, there was no improvement. Vitamin B12, Folate and Iron are normal. I quit smoking 1 year ago (6 years history). Please guide regarding the possible cause and proper treatment.

Care AI Shimmer
Hello, Based on your history (burning sensation for 1 year, worse in evening, white line/patch-like changes, sensitivity to spicy food, normal blood reports), the possibilities include: • Oral Lichen Planus (OLP) • Oral lichenoid reaction • Early Burning Mouth Syndrome (if no significant clinical lesion) Since symptoms are persistent for 1 year and not responding to routine treatment, this should not be ignored. I would strongly recommend: • A detailed clinical examination by an Oral Medicine specialist • If required, an incisional biopsy to confirm diagnosis • Evaluation for any local irritants (sharp teeth, restorations, tobacco history) Avoid spicy food, alcohol-based mouthwash, and stress triggers. Please upload a clear close-up picture of the affected area for better guidance. Proper diagnosis is essential before further treatment.
