Dear Patient, Thank you for sharing your detailed medical history. Your symptoms—**persistent painful bruises, difficulty gaining weight, fatigue, and abnormal coagulation markers**—warrant a thorough evaluation. Here’s my analysis:  Key Concerns: 1. Bruising & Coagulation Abnormalities:    - Your **PT/INR (1.24)** and **APTT (35.92)** are near-normal, but the **PT INR (patient value) of 15.2** is unclear (possibly a typo). Clarification is needed.     - Bruising suggests possible **clotting factor deficiency, platelet dysfunction, or liver-related issues**.  2. Gilbert’s Syndrome & Liver Health:     - Mildly elevated **bilirubin (2.30)** aligns with Gilbert’s, but rule out other liver conditions (e.g., fatty liver, hepatitis).     - **A/G ratio (1.96)** is normal, but your **low weight (37 kg)** raises concerns about malnutrition or malabsorption.  3. Weight & Fatigue:     - Could stem from **nutrient deficiencies (e.g., vitamin K, B12)**, chronic inflammation, or metabolic disorders.  Urgent Next Steps: - Tests Required:   - Complete blood count (CBC), liver function tests (LFTs), vitamin K/B12/folate levels.    - Clotting factor assays (e.g., factors II, VII, IX, X) and platelet function tests.    - Ultrasound/elastography to assess liver structure.  - Immediate Actions:   - Avoid NSAIDs or activities that increase bruising risk.    - Eat small, nutrient-dense meals (prioritize protein, healthy fats).  Consultation Recommended: Your case requires specialized attention to diagnose the root cause (hematological, hepatic, or nutritional). Painful bruises, low weight, and tiredness may be due to: Vitamin K or C deficiency Mild liver-related clotting issue Rare clotting/platelet disorder Gilbert’s syndrome usually doesn’t cause bruises, but it may add to the problem Suggested tests: CBC, LFT, Vitamin K/C/D, B12, iron, thyroid Start a high-protein, calorie-rich diet and take multivitamins Please consult me directly for proper evaluation and treatment I handle similar cases regularly and can guide you step by step Book a consult with me on Practo today for expert care.
