Hello. I have reviewed your inquiry regarding how to identify and diagnose Irritable Bowel Syndrome (IBS). IBS is primarily a "diagnosis of exclusion," which means doctors often perform tests to rule out other issues before confirming it is IBS. 1. How IBS is Identified Doctors look for a specific pattern of symptoms, including: Abdominal Pain: Usually related to bowel movements. Changes in Stool: Alterations in how often you go or the consistency (texture) of the stool. Bloating: A frequent feeling of fullness or gas. 2. Necessary Tests and Diagnosis To reach a diagnosis, a physician may recommend the following to rule out infections or inflammatory conditions: Stool Analysis: To check for infections, parasites, or hidden blood. Blood Tests: Often including a Complete Blood Count (CBC) and tests for C-reactive protein (to check for inflammation) or Celiac disease screening. Breath Tests: Sometimes used to check for Small Intestinal Bacterial Overgrowth (SIBO) or lactose intolerance, which can mimic IBS. Imaging: In some cases, a colonoscopy or CT scan may be needed if "red flag" symptoms (like weight loss or bleeding) are present. Recommended Next Step You should consult a Gastroenterologist. They will review your full history and determine which specific tests are necessary for your case.

Basically IBS is a diagnosis of exclusion. Detailed history and evaluation is needed to rule out any other pathology. If you have IBS a detailed mental status examination will also help as probiotics or antibiotics won't help much because IBS occurs because of some unaddressed mental stress/anxiety/ overthinking/ depression/ suppressed trauma. Please consult for a detailed evaluation and management plan. I provide online consultation and therapy support.

Irritable Bowel Syndrome (IBS) is mainly a clinical diagnosis — there is no single confirmatory test. 📌 How to identify IBS? Doctors use Rome IV criteria: Recurrent abdominal pain (≥1 day/week in last 3 months) Associated with ≥2 of: Related to defecation Change in stool frequency Change in stool form 🔍 Tests (to rule out other causes) CBC, ESR/CRP Thyroid function test Stool routine (infection, parasites) Celiac screening (if indicated) Colonoscopy only if red flag signs present ⚠️ Red flag signs (NOT IBS → need evaluation) Weight loss Blood in stool Anemia Nocturnal symptoms Family history of cancer/IBD 💊 Probiotics / antibiotics? Probiotics may help symptoms in some patients Antibiotics not routinely needed (only in selected cases like suspected SIBO) ✅ Bottom line IBS is diagnosed based on symptoms + exclusion of serious disease, not by a single test.

IBS (Irritable Bowel Syndrome) is diagnosed mainly based on symptoms, after ruling out other diseases. There is no single confirmatory test. Common symptoms: * Recurrent abdominal pain * Bloating * Change in bowel habits (diarrhea / constipation / both) * Pain related to passing stool Tests done to rule out other causes: * Blood tests (CBC, ESR/CRP) * Thyroid function test * Stool examination (infection, parasites, occult blood) Red flag symptoms (need urgent evaluation, not typical IBS): * Weight loss * Blood in stool * Fever * Night symptoms * Family history of colon cancer Treatment point: * Probiotics may help * Antibiotics are not routinely needed unless a specific condition is diagnosed
Next Steps
* Consult for evaluation * Avoid self-medication * Track food triggers and stress * Diet and lifestyle changes are key

Irritable Bowel Syndrome (IBS) is fundamentally a diagnosis of exclusion. This means there is no single blood test or scan that 'identifies' IBS. Instead, a physician must first rule out organic diseases—such as Celiac disease, Inflammatory Bowel Disease (IBD), or infections—through specific tests. Only when these tests are normal and the patient meets the Rome IV Criteria (recurrent abdominal pain and changes in bowel habits) can a definitive diagnosis of IBS be made
Health Tips
IBS is about how the gut functions, not how it looks. Book a consultation so we can run the necessary screening tests to ensure your gut is structurally healthy and design a diet and stress-management plan tailored to your symptoms."
