BP fluctuating high
Since 2-3 days i have been recording bp at home/ since i am getting headaches, ( at back of head n neck) palpitations and general uneasiness- Bp stays 136/140: 85-95 most of the times- Only in morning it would be 120/80 I am already on telma 40 mg n i am scared with the numbers now- Does this require checkup at clinic or i can manage it by rest, hydration and less sodium ( my daily intake goes 7-8k)

Your readings: • Morning: 120/80 (excellent) • Rest of day: 136–140 / 85–95 • On Telma (Telmisartan) 40 mg These values fall into: • High-normal to Stage 1 hypertension range • Not an emergency • Not dangerous in the short term Important observations: ✔ Morning BP normal → medication is working ✔ Mild daytime elevation → likely due to: • Stress • Headache itself • Anxiety • Heat/dehydration • Excess sodium intake (7–8g/day is high) • PMS phase (you mentioned earlier) Your symptoms (occipital headache + neck pain + palpitations + uneasiness) are very typical of anxiety-driven sympathetic overactivity, especially when someone keeps checking BP frequently. BP of 140/90 does NOT usually cause headache unless it is >160–170 systolic. Right now, this does not sound like a hypertensive emergency. ⸻ Do You Need Clinic Visit? You can monitor at home IF: • BP stays below 150/100 • No chest pain • No breathlessness • No visual disturbance • No neurological symptoms Visit clinic if: • BP persistently >150/95 despite rest • Headache severe and not responding • Palpitations sustained >110 bpm at rest • Anxiety becoming overwhelming For now, I would advise: • Do NOT check BP more than twice daily • Sit calmly 5 minutes before measuring • Avoid measuring during headache or stress episode Overchecking increases adrenaline → raises BP → increases anxiety → vicious cycle.
Next Steps
Immediate plan (next 5–7 days): ✔ Reduce sodium to
Health Tips
BP naturally fluctuates 20–30 mmHg throughout the day. A reading of 138/90 in a stressed person is not dangerous. Your morning normal BP is the most reassuring sign. ⸻ If after 1 week of lifestyle correction your average BP remains >135/85, then we can consider: • Dose timing adjustment • Adding low-dose second agent Since you are already on treatment, I would suggest a structured review rather than panic-driven changes. If you’d like, share your 7-day BP log and we can analyze trends properly and decide whether medication adjustment is truly needed.

Your BP readings are mildly elevated but not emergency levels. Stress, anxiety, and headache can temporarily increase BP. Morning readings being normal is reassuring.
Next Steps
Reduce salt, rest well, stay hydrated, and monitor BP twice daily for a few days. If readings stay high or symptoms persist, please consult me for proper diagnosis and treatment.
