Black blood during periods normally means that the blood is taking some time to leave the uterus after ot has been shed from the Endometrial lining. Normally the blood doesn't stay in the uterus for any length of time.
If blood stays in the uterus then it has time to undergo oxidation and the haemoglobin develops black colour.
It could also be due to bacterial vaginosis and trichomoniasis , both are vaginal infections but invading the uterus.
Pt should have a D&C and after that given treatment for bacterial vaginosis and trichomoniasis.
