Black large hole visible in follic scan
I am 25 yr old girl. My blood reports were as under : Amh : 1.17 Fsh : 9.5 Lh : 3.6 HPLC - N Also in May 2020 my Tsh was 6.2 after which i started 25 mgs of tablet daily for thyronom. I am trying to conceive since an year. So i went for follicular monitoring wherein it is observed that there is a big fluid filled gap in the right side of ovary. Though good follicules were formed and ruptured from left side. Doctor is not sure if it is an ovarian cyst only or also a tubal blockage. Is it possible if my one tube is blocked other may also not b functioning properly ? Do i need to go for laparoscopy? Please suggest. (Follicular scans attached) Please suggest.

Dear Practo User, there are a number of issues. First is if you have hydrosalpinx, that tube is unhealthy. The fluid collected is also bad for your embryos. So this needs to be managed first. Very likely the other tube will also be damaged. Secondly, if you are just 25yrs, your AMH is quite low. With all these factors conceiving can be a problem. Firstly don’t worry . Go for a scan on day 2/3 of your period —-this will clarify if you have cyst and how many Antral follicles you have in each ovary . Next again repeat a scan in between day 12-14 of your cycle , they will check and let you know whether you have fluid in your tube (hydrosalpinx) .yes , chances are that if one tube is damaged the other may be.

If u are getting difficult in conceiving than plan for diagnostic laproscopy   followed by theraptic  . It clear the situtation and get specimen for cultre for tuberculosis and biopsy. Hydrosalpinx shd be treated.

Avoid stress and anxiiety That's the 1st step to be avoided Hydrosalpinx needy treatment first as the fluid present in it is not good for embryo to grow! That can be treated conservative/surgical procedure as per patient and your GYNAECOLOGIST choice

Hydrosalpinx needs to be draining laparoscopically otherwise it will get infected and prevent implantation
