Anal fistula mri
Does anal fistula mri show track only when active abcess is present or it shows even if I don't have abcess at present.my mri is showing no track since 6 months on repeated mri.

Hello Practo user If the abscess is present then only MRI shows track but there are chances or recurrence even after there is no present abscess. To stop the recurrence consult a Homoeopathic doctor and take proper medications

Hai, Its very interesting question. Anal fistula MRI is MRI FISTULOGRAM. Its a gold standard to visualise fistula, both active ( with abscess ) and inactive ( no abscess, healed tract with fibrotic margins ) cases. In MRI, fistula with abscess, take high signal so it will appear in white colour. In MRI, fistula without abscess will take low signals so it appear as dark coloured tract. If MRI is showing no tract, that it means fistula is cured. Fistula is a localised disease that have tendency to recurr so take medicines accordingly.

Dear Mam An MRI fistulogram does not require an active abscess to show a fistula tract. A good-quality pelvic MRI will normally pick up even a dormant, non-infected fistula tract because the tract is made of fibrous tissue it doesn’t disappear simply because the abscess is inactive. So if your repeated MRIs over the last 6 months are showing no fistula tract, this is actually a very reassuring sign. However, symptoms like pain, discharge, swelling, itching, or repeated infections can sometimes happen even when the main tract has healed. These may be due to: A previously healed fistula site that is still sensitive Residual inflammation Anal fissure Skin infection Recurrent constipation or IBS Pelvic floor muscle spasm Post-surgical scar tissue These conditions can mimic fistula pain, so it’s important to evaluate them properly. If MRI is not showing any track or active abscess, May be it is not present at the moment. But it has a tendency for recurrence. So be careful and watchful.

Even without an active abscess an unhealed fistula will show a track on MRI. Your MRI suggests that the fistula no longer exists. For all practical purposes, if a high-quality MRI shows no evidence of an active fistula tract, you should consider the fistula healed. Your doctor who examines you will combine the MRI results with the following Clinical Healing factors to confirm success: Cessation of Discharge: No pus, blood, or serous discharge from the external opening for an extended period (usually 3 to 6 months). Absence of Symptoms: No pain, swelling, or localized tenderness in the area. External Opening Closure: The external wound has fully closed or is fully healed

However a very small, residual cluster of inflammatory tissue or a microscopic tract may sometimes remain, which is too small for even the MRI to detect clearly. An MRI can detect fistula tract, internal opening, secondary branches & associated abscess because MRI uses powerful magnetic field to detect any abnormal tract or any formation in human body. If your MRI is showing no active tract then it means the tract is completely healed and if you still feel any symptoms beside this normal MRI then there may be a chance of improper MRI examination.
