Does anal fistula mri show track only when active abcess is present or it shows even if I don't have abcess at present.my mri is showing no track since 6 months on repeated mri.

If the abscess is present then only MRI shows track but there are chances or recurrence even after there is no present abscess. To stop the recurrence consult a Homoeopathic doctor and take proper medications

Its very interesting question. Anal fistula MRI is MRI FISTULOGRAM. Its a gold standard to visualise fistula, both active ( with abscess ) and inactive ( no abscess, healed tract with fibrotic margins ) cases.
In MRI, fistula with abscess, take high signal so it will appear in white colour.
In MRI, fistula without abscess will take low signals so it appear as dark coloured tract.
If MRI is showing no tract, that it means fistula is cured.
Fistula is a localised disease that have tendency to recurr so take medicines accordingly.

An MRI fistulogram does not require an active abscess to show a fistula tract.
A good-quality pelvic MRI will normally pick up even a dormant, non-infected fistula tract because the tract is made of fibrous tissue it doesnât disappear simply because the abscess is inactive.
So if your repeated MRIs over the last 6 months are showing no fistula tract, this is actually a very reassuring sign.

If MRI is not showing any track or active abscess, May be it is not present at the moment.
But it has a tendency for recurrence.
So be careful and watchful.
Even without an active abscess an unhealed fistula will show a track on MRI. Your MRI suggests that the fistula no longer exists.
For all practical purposes, if a high-quality MRI shows no evidence of an active fistula tract, you should consider the fistula healed.
Your doctor who examines you will combine the MRI results with the following Clinical Healing factors to confirm success:
Cessation of Discharge: No pus, blood, or serous discharge from the external opening for an extended period (usually 3 to 6 months).
