You would require extraction.Do visit an oral surgeon or dentist as soon as possible.
Meanwhile apply ice packs on your cheek to help your pain and rinse with salt on lukewarm water after every meal.

Third molar looks decayed, infection and closeness to the nerve can be a cause of pain.
Better to visit oral surgeon and get a CBCT done to check 3D view of the third molar, this will tell you about infection in the tooth and closeness to the nerve in millimetre.

Hello. This X-ray was shot in July. Nevertheless, there is a very small abscess near the root tip of your left lower 3rd molar- probably due to a cavity that can be seen in that.
Also your right upper 3rd molar seems to be decayed as well.
Other than this I can see a root piece/odontome in between your lower right premolars. Are you sure that is not causing the radiating pain?
Please do consult an oral surgeon and get a NEW OPG done to assess the latest developments,(perhaps the abscess might have become bigger.)
U can hv extraction done the side which is paining more..Because both side wisdom teeth are impacted ..They need extraction...Even after extraction u hv pain than consult neurologist
Dear Lakshmi
It would be better if you consult an oral surgeon to evaluate your problem and discuss the treatment plan.
Definitive diagnosis is important before any treatment is initiated.
We have to analyse the condition in more detail, because the x-ray is not full fledged.
Both 38 and 48 appears to be near to the inferior mandible canal . Also radiolucencies associated with 38 and 48 . Close section of cbct of 38 and 48 is required . U need to get them removed surgically by an efficient oral surgeon .
