White head on nipple. Be contacted?

Hi My father has a white head on nipple and there is mild pain around that area which fades on its own and this is there since last 10 March and he was recovering from covid that time. Around 2nd March  he had an x ray chest because of covid monitoring and that was fine. He had an ultrasound 4 days back and report was all fine with no cyst or anything to worry. Pulmonologist suggested tab Augmentin 625 for 2 times for 8 days but problem is same. Physician suggested Nurokind LC, Vitamin D, calcium in morning along with Rozavel 10 in night but that did not helped too. General surgeon said it might be discoloration since other findings by previous doctors did not revealed anything. The white head is still there .no pus feeling, no lumps feeling. White head is as it is from 1 month and faiding mild pain is there too. Please suggest what can be done , whom should be contacted ? PS: All consultations were done via video and ultrasound doctor only examined the white head Physically.
