India has a huge problem of vitamin D deficiency! Vitamin D 60 k once a week for 8 weeks should be good for now. Metformin can cause reduced Vitamin B12 levels so supplements are always recommended!

At HBA1C of 6.4 you can give a trial of dietary and lifestyle modifications for three months. Repeat HBA1C after 3 months and if it persists we’ll have to opt for detailed consultation and start medication. As for Vit D levels you should start D3 60,000 IU once a week for 12 weeks and try to increase the sun exposure.

An HbA1c of 6.4% is at the borderline between prediabetes and diabetes. Prescribing Galvus Met 50/500 for 3 months is a reasonable and preventive approach, especially if you have risk factors like weight gain or family history. This helps improve insulin sensitivity and may prevent progression. Your Vitamin D level of 8.48 ng/mL is severely deficient. This does need treatment, as such low levels can contribute to fatigue, body aches, low immunity, mood changes, and can also worsen insulin resistance over time.
Next Steps
• Start Vitamin D replacement: • Common regimen: Vitamin D3 60,000 IU once weekly for 6–8 weeks, followed by a maintenance dose (as advised by your doctor) • Continue Galvus Met as prescribed • Recheck HbA1c and Vitamin D after 3 months • A general physician or internal medicine doctor is appropriate for managing both issues; no separate specialist is required at this stage
Health Tips
• Take Vitamin D after a meal containing fat for better absorption • Get 15–20 minutes of sunlight exposure when possible • Maintain regular physical activity and weight control • Avoid excess sugar and refined carbohydrates • Do not stop or adjust medicines without medical advice I know seeing multiple abnormal values together can feel overwhelming. The reassuring part is that both conditions are very common and very treatable, especially when addressed early—as you’re doing now. With a bit of guidance and follow-up, these numbers can improve significantly. If you’d like, an online consultation can help fine-tune your treatment and lifestyle plan so you feel confident and supported throughout.

HbA1c 6.4 = early diabetes (Galvus Met is appropriate). Vitamin D 8.48 = severe deficiency — this can cause fatigue, body aches, low immunity. Start Vitamin D3 60,000 IU once weekly for 6 weeks, then monthly maintenance.
Next Steps
Continue Galvus Met as advised. Add daily walking + reduce sugar/refined carbs. Repeat HbA1c after 8–12 weeks. A general physician is enough for now.
Health Tips
I understand this feels stressful — but both issues are very common and fully manageable. If you’d like, book a consult with me — I’ll personalise your plan and follow you up properly.
