Upper back pain
April 2025 srirama navami I lifted talambralu rice 7 kgs rice by hands placed between in front chest area by plate  and walked to temple almost 40 mints that time onwards full upper back pain .used etoricoxib and thiocolchicoside tablets no relief from pain.and used  gabapentin 300mg tablets no relief from pain what type of physiotherapy can u suggest.plz reply.what tablet can I use .what physiotherapy can u suggest.plz reply.is it because of weight lifting caused this pain.mri thoracic spine normal.

As u described pain likely started due to sudden heavy lifting and prolonged walking with load, affects the upper back muscles, scapular Good part MRI of the thoracic spine is normal

First focus on pain relief -  (DOES) 01- Apply hot formantation on affected part  10min /2 times daily 02- rest for 15 days (can do your day-to-day activity but obey dos and don'ts 03- rolle  one long towel or bedsheet and place it under your neck ( sleep in supine ) , hold it for 15 min / 2 times daily Do this all for 15-20 days

Patient Complaint: Persistent upper back pain since April 2025 after lifting 7 kg of rice (talambralu) and walking ~40 minutes. Pain continuous since then. No relief with Etoricoxib, Thiocolchicoside, or Gabapentin. MRI Thoracic Spine: Normal ⸻ Clinical Impression / Diagnosis: • Upper back myofascial strain involving trapezius, rhomboids, and thoracic paraspinal muscles • Postural overload due to prolonged weight carrying in front of the chest ⸻ Physiotherapy Treatment Plan: 1. Pain Relief Phase (First 7–10 days) • IFT / TENS therapy: 15–20 min for pain relief • Ultrasound therapy: 7–10 min on affected thoracic and interscapular area • Moist heat / hot pack: 15 min before exercises • Gentle soft tissue & myofascial release for trapezius and rhomboids ⸻ 2. Recovery Phase (After pain reduces) • Postural correction exercises • Scapular setting & strengthening: • Scapular retraction (shoulder blade squeeze) – 10 reps × 3 sets • Wall slides / Wall angels – 10 reps × 3 sets • Thoracic extension over rolled towel – 10 reps • Cat-camel stretches for spinal mobility • Breathing exercises to relax thoracic muscles ⸻ 3. Ergonomic & Lifestyle Advice: • Avoid lifting heavy weight close to the chest for long duration • Maintain upright posture while sitting or standing • Support back with cushion while working or driving • Gentle stretching every 2 hours if working long at desk ⸻ Medication Advice: Continue only mild pain-relief as prescribed by physician for short term. Focus on physiotherapy and exercises for long-term recovery. ⸻ Prognosis: Good.
