Uneven thigh creases in infants can sometimes be a normal variation.If the baby moves both legs normally and there is no restriction, it may be a normal soft tissue variation, but a physical examination is recommended to rule out hip dysplasia.

Ultrasound of hip advised to evaluate the hip joint.
As right leg is in external rotation and left leg in neutral position , it might be normal and it also depends on soft tissue around thigh.
If the pt is able to flex the limb fully and extend it without pain then it can be normal
If there is any kind of pain and the child becomes irritable then the child should be shown to a near by orthopaedics doctor so he can evaluate it physically.
