Ultrsound and FNA for thyriod
A 5×4mm nodule finds In left part of thyriod...after FNA it falls under bethesda 3 nodule..it mentioned that cell is round to oval shape with hyperchromatic nuclei, scanty cytoplasm arranged in small cluster and scattered singly against back ground of hamerroge..I attached the photo of report..plz explain

Right thyroid Nodule - FNAC - Bethesda II - CAN WAIT. Left thyroid nodule - FNAC - Bethesda III and TIRADS IV.  FNAC says atypia of undetermined significance. The chance of it being cancer is 15% if cytoplasmic atypia and 45% in case of nuclear atypia. The details is not mentioned in the report. Your Ultrasound says calcifications in the nodule and there is a node in level VI. With all these features my suggestion would be.. 1. Left hemithyroidectomy - wait for the histopathology after surgery and decide of completion thyroidectomy. If not proceeding with completion, then follow up on right Lobe every six months. 2. Total thyroidectomy with central compartment clearance if frequent follow up is not possible. 3. Wait and repeat USG GUIDED FNAC after six weeks. Most likely will be the same report and we will be in this same place at that time too.
Next Steps

This report is not satisfactory as Fnac was likely done without ultrasound guidance, u will need to repeat ultrasound guided fnac

This is ultrasound guided FNAC only and no need to repeat FNAC again. Get fT3, fT4, TSH done. This patient must under hemithyroidectomy depending upon his thyroid profile as patient lies in high risk age group with multiple nodule
