There is a 49X58X57.5mms(SI,AP,TR) cyst (dermoid cyst) in right adnexa with fat density - 120 H.U,peripheral calcification and soft tissue density within it. However no significant enhancement is seen after IV contrast.
Do i need to operate this immediately?

Are you married. If you are planning pregnancy then better remove...or if you are still unmarried go for surgery. Cyst smaller than 10 CM do not require urgent surgery...but dermis will OT go by medical treatment
