Hi,
You should start medicines for hypothyroidism along with healthy dietary and lifestyle changes. Once it is properly controlled there should be no issues conceiving.
Hello,
I am Dr. Romain Rajan, General Physician (Ex Govt Medical Officer at RML Hospital, New Delhi), and your case has been allotted to me.
Thank you for sharing your details. I understand your concern regarding thyroid, low weight, and planning for pregnancy, and I will explain this clearly.
About thyroid and conception
If TSH is high, it means your thyroid is not optimally controlled.
Poorly controlled hypothyroidism can affect ovulation and fertility, but the good news is that once TSH is properly controlled, chances of conceiving are usually good.
For women planning pregnancy, the ideal TSH target is below 2.5.
About low weight
Being underweight can also affect hormonal balance and ovulation, but it does not mean you cannot conceive. Many women conceive successfully once thyroid levels and nutrition are corrected.
Low weight in your case may be due to:
⢠Inadequate calorie intake
⢠Poor absorption
⢠Long standing thyroid imbalance
⢠Stress or irregular eating patterns
What you should do now
⢠Continue thyroid medicine regularly (do not stop or skip doses)
⢠Get repeat TSH, Free T3, Free T4 tested
⢠Dose adjustment may be needed to bring TSH below 2.5
⢠Take thyroid tablet on an empty stomach, avoid food for 30 minutes
How to gain weight safely
⢠Eat small, frequent meals
⢠Increase protein intake like milk, curd, paneer, eggs, pulses
⢠Add healthy calories like nuts, seeds, ghee, and oils
⢠Do light strength exercises to build muscle mass
⢠Avoid crash dieting or skipping meals
When to consult a doctor
⢠If TSH remains high despite medication
⢠If periods are irregular
⢠If you are planning pregnancy actively
With proper thyroid control and gradual weight improvement, most women conceive normally.
For personalised one to one consultation, thyroid dose adjustment, and pregnancy planning, you can reach out to me directly on WhatsApp at 85271646 seven seven.
Wishing you good health and the best ahead.
It looks like subclinical hypothyroidism because T3 and T4 were normal. Repeat the thyroid test after 3 months.
Hypothyroidism causes weight gain not weight loss. Also please check for autoimmune cause do Anti Tpo test to rule out autoimmune cause.
For not gaining weight take Multivitamin tablet and protein fiber rich diet.
Sometimes hypothyroidism cause infertility. Repeat the test once again and check for autoimmune cause if still tsh is high consider taking thyroid supplement
As per your report you are hypothyroid.
Hypothyroidism clinically presents as weight gain mostly.
But your complaint is not gaining weight.
Better you get repeat test of thyroid funtion done as report and symptoms are not matching.

High TSH indicates hypothyroidism, which can affect weight and sometimes delay pregnancy, but the good news is that once thyroid levels are properly controlled, most women conceive normally. For women planning pregnancy, TSH should ideally be below 2.5, so regular treatment with levothyroxine (thyroid medicine taken daily on an empty stomach) is important and the dose should be adjusted only after seeing recent TSH and Free T4 reports. Low body weight can also affect ovulation, so gradual weight gain with a balanced diet containing adequate calories and protein (milk, curd, paneer, nuts, fruits, regular meals) is advised. Please get your thyroid tests repeated if not done recently and consult a physician for correct dose adjustment, as proper control of thyroid greatly improves health, weight, and chances of conception.
With high TSH and normal T3/T4, this suggests subclinical hypothyroidism. If TSH is persistently above the recommended range , it can affect ovulation and fertility, but this is reversible with proper treatment.
What to do:
Consult a physician/endocrinologist for low-dose levothyroxine, aiming for TSH within normal limit before conception.
Check anti-TPO antibodies, as autoimmune thyroid disease can impact pregnancy.
Regularly monitor TSH every 6–8 weeks.
Weight gain advice:
Eat frequent, calorie-dense meals (milk, curd, nuts, peanut butter, ghee).
Ensure adequate protein intake.
Avoid unnecessary dose escalation of thyroid medicine.
Please share your height, whether one is of normal weight or underweight can be decided by BMI.
Usually hypothyroidism causes weight gain, please share your latest results, then accordingly dose of medicine can be adjusted. Share what medicine in which dose, you are taking?
Take food with more proteins, healthy carbs, like beans, lentil, soya, brown rice, quinoa, oats, berries, sweet potatoes.
Regular exercise will be helpful

I don’t think your Thyroid is the reason for not gaining weight. Some people are well and just have low weight.
Do the following,
1. Take Thyoronorm daily (as your blood test suggests your thyroid is not adequately treated and can cause problems either getting pregnant or in pregnancy) without missing doses
2. Blood tests- CBP, ESR, HbA1C
All the best.
