Thyroid levels and medication
My TSH is 2.58 as on October 2025. I am taking Thyronorm 50 mcg everyday for the past four years. Should I continue with the same medication or reduce the dose? Kindly advise. Thanks.

TSH 2.58 = well controlled. Continue Thyronorm 50 mcg daily at the same dose. Do NOT reduce or stop unless advised after repeat testing. Repeat TSH every 6–12 months (earlier if symptoms appear). Dose reduction is considered only if TSH is persistently low (<0.5) or hyperthyroid symptoms develop.

Your TSH level is normal at 2.58. Since you have been taking 50 mcg thyroxine for 4 years and your report is stable, you should continue the same dose. No change is needed now. You only need to repeat the TSH test if you develop symptoms such as tiredness, weight gain or weight loss, feeling very cold or hot, hair fall, swelling of face or legs, fast heartbeat, or anxiety.

Hello, I am Dr. Romain Rajan, General Physician (Ex Govt Medical Officer at RML Hospital, New Delhi), and your case has been allotted to me. Thank you for sharing your report. I understand your concern regarding long term thyroid medication, and I will explain this clearly. Based on the information you have shared, a TSH value of 2.58 while taking Thyronorm 50 mcg daily indicates that your thyroid levels are well controlled on the current dose. At present, this value is within the target range, and it suggests that the medication dose you are taking is appropriate for your body. Should you reduce the dose At this stage, dose reduction is not advised unless: • You are having symptoms of excess thyroid hormone like palpitations, anxiety, tremors, unexplained weight loss, or heat intolerance • TSH consistently drops below the normal range on repeat testing Reducing the dose unnecessarily can lead to rise in TSH again and recurrence of symptoms like fatigue, weight gain, hair fall, and low mood. What you should do now • Continue Thyronorm 50 mcg once daily as prescribed • Take it on an empty stomach with water • Avoid food or other medicines for at least 30 minutes after intake • Repeat TSH after 6 to 12 months, or earlier if symptoms appear When to consult a doctor • If you develop new symptoms suggestive of over or under thyroid • If TSH values change significantly on follow up • If you are planning pregnancy or have major weight changes Please remember that stable thyroid control over years is a good sign, and long term continuation at the same dose is very common and safe when monitored properly. If you wish to discuss your reports in detail, symptoms, or long term thyroid planning, a direct consultation can help personalise your care better. For personalised one to one consultation on this case or any other health concern in the future, you can reach out to me directly on WhatsApp at 85271646 seven seven. A detailed discussion allows better assessment and long term peace of mind. Wishing you good health. Dr. Romain Rajan General Physician
