Testicular varicocele and tight frenulum

Asked for Male, 22 Years

I have left testicular varicocele and tight frenulum in the penis. Regarding varicocele, although it's relatively painless i'm more concerned about potential negative impact it may cause with respect to sperm quality and testosterone volume. Also regarding frenulum, when i pull back my foreskin it causes a dip in the penis head . What Should be the appropriate course of action to the remedy these two cases ? P.S: i'm attaching picture of left varicocele and tight frenulum , can you guess like , on first impression, which grade of varicocele is this ?
