Testicle pain since 2 year

Asked for Male, 28 Years

Sometimes my right testicle have mild pain ,it's happening since 2 years. Why? Along with testicle pain, I also get pain in my right lower abdominal. Sometimes the right testicles shrinks but may not along with left testicles n looks litle smaller as compare with the left testicle. (Attached it's picture). Today I did usg of scrotum(I HV attached)... Please help me to know why I am getting pain ,how to cure it. Thanks..
