Telmisartan vs Cilnidipine — which is better?
Both medicines are used for high blood pressure, but they belong to different classes and work in different ways:
1. Telmisartan – an Angiotensin Receptor Blocker (ARB)
Protects heart, kidneys, and blood vessels long-term.
Especially beneficial if you have diabetes, borderline sugar, kidney disease, or obesity.
Has additional metabolic benefits (slight improvement in insulin sensitivity and cholesterol).
Usually well tolerated with very few side effects.
2. Cilnidipine – a Calcium Channel Blocker (CCB)
Helps relax blood vessels and reduces BP effectively.
Also reduces heart rate and prevents reflex tachycardia (better than older CCBs like amlodipine).
Sometimes causes mild ankle swelling or leg heaviness with long use.
Does not protect kidneys as strongly as Telmisartan.
Long-term preference
If you are a young male (37 years) with no major kidney, heart, or diabetic issues, Telmisartan is generally considered better for long-term control and organ protection.
In fact, many modern BP regimens use a combination — Telmisartan + Cilnidipine — for patients whose BP is not well controlled by one alone.
So, the choice depends on:
Your average BP readings
Any swelling in feet (common with Cilnidipine)
Kidney/liver function
Presence of diabetes or obesity
Suggested plan
1. If you’re newly diagnosed and BP is mildly raised (≤150/90) — start with Telmisartan 40 mg once daily.
2. If BP remains above 140/90 after 10–14 days — your doctor can add Cilnidipine 10 mg to the same tablet (fixed-dose combo available).
3. Always monitor BP twice daily for 7 days before deciding to change medicine.
4. Continue lifestyle modification:
Reduce salt (<5g/day), avoid processed food.
30 min brisk walk daily.
Maintain healthy weight, avoid stress and alcohol.
Reassurance
Both drugs are safe for long-term use, but Telmisartan offers better heart and kidney protection and fewer side effects — so it’s preferred as the base medicine.
However, final adjustment should be made after reviewing your BP trend, kidney function, and pulse rate.

For young age hypertension, Telmisartan is the preferred drug because it has protective effect on kidneys and heart.

As per guidelines
Restrict Salt intake to less than 5 gm per day
Exercise at least 5 times a week for 20 to 30 mins
Monitor your Blood Pressure regularly
Do not stop medications if Blood pressure comes back to normal.

Both are useful, both have side effects, both have some protective function for the heart.
You can continue one of them, in prescribed dose ,till you tolerate it well. If any side effect develops, can be changed.

Considering your age as a factor , Telmisartan is the better choice of drug for your hypertension
