I was detected by scanty tb so my medicines were started my weight is 80 kg so doctor said me to take 6 tablets each consisting of rifampicin 150mg,isoniazid 75mg , pyrazinamide 400mg,ethambutol 275mg. After 2 months I have gone to different doctor he reduced every drug and said this is overdose and long term very harmful so he gave rifampicin 600mg, isoniazid 300mg,ethambutol 1000 mg. Is it right and will it cure remaining bacterias

Drug sensitive tb treatment traditionally consists of 6 months ATT, which includes 2 months of intensive phase with isoniazid, rifampicin, pyrazinamide, ethambutol and continuation phase of 4 months with isoniazid , rifampicin and ethambutol….. government medicines come as fixed dose combinations based on weight band where as private medicines come seperately…in your case either of the doctors can’t be said wrong, as literature says maximum tolerable dose of ethambutol to be taken as 600 mg , but if we prescribe based on fixed dose combinations weight band, ethambutol dose goes much higher… and the reason why dose was reduced after 2 months is - you were shifted from intensive phase to continuation phase..

In treatment of tuberculosis initially 4 drugs are given that is isoniazide, rifampicine , pyrizinamide and ethambutol. After 2 month pyrizinamide is not given..so you will be prescribed isoniazide, rifampicine and ethambutol is given...i think isoniazide and rifampicin is fine but ethambutol should be given minimum of 1200 mg ( if 80 kg weight)
Next Steps
if you are clinically improving nothing to be done...you can continue with this regimen if symptoms are improving..no need to worry

Pl consult tb centre. The drug’s are given free and of standard quality. The dosage of tb medicines are wt dependent. Pl take regularly.

After 2 months pyrazinamide will be stopped The individual dosage is not approprite
