Your father has TB lymphadenopathy and is now suffering ATT-induced hepatitis. In such cases, the anti-TB treatment (ATT) regimen needs to be tailored carefully, as the standard medicines are affecting liver. Weekly monitoring of liver function tests (LFTs) is important until the values normalize. Since he has a history of cardiac stenting, it’s very important to know about his other comorbid conditions and current medications before modifying the TB regimen. He may consult me for an appropriate tailored ATT regimen and close monitoring plan.
Next Steps
Stop all TB medicines for now (under doctor’s supervision and depending on LFT REPORT), monitor LFTs weekly, and once the liver stabilizes, we can gradually reintroduce or substitute medicines based on tolerance and cardiac profile.
