If the fissure didnt resolve even after 2 surgeries ,then its not a Normal fissure .. U may be having some different underlying contion which is presenting as one of the symtpoms as fissure.. It may be Crons or IBD or  others.. U need detailed evaluation.
Next Steps
Kindly consult a Gastroenterologist for further evalutation of ur colon .. With adequated sedation they will do ur colonoscopy +/_ biopsy and evalutate ur condition. Until then continue with ur laxatives local Lignocaine 2% gel application Diltiazem 2% gel  (CCB) Sitz bath in Povidine For further details u can consult me on Practo

Hi! Don't worry, chronic fissures can be managed well even after surgeries. 😊 Pain and incomplete evacuation improve with diet, better stool regulation, and advanced care options. Feel free to consult for a personalized plan to help you. 👍

A chronic fissure persisting for 5 years with two failed surgeries at age 22 is a red flag — this pattern strongly suggests an underlying condition like Crohn's disease, which causes non-healing refractory fissures that don't respond to standard surgery. The fragmented incomplete stools with pain also suggest possible dyssynergic defecation (pelvic floor dysfunction), which requires biofeedback therapy, not just stool softeners.
Next Steps
Please see a gastroenterologist for a colonoscopy to rule out Crohn's disease and anorectal manometry for pelvic floor assessment. Feel free to consult me directly on Practo for a detailed evaluation.
