Hi doctor, we have observed sudden twitch of upper lip from past one month while he is talking and when he is sleeping he is ok. Please advice

This kind of upper-lip twitching is most commonly a benign muscle twitch (habit spasm or transient tic). It is usually related to fatigue, stress, excitement, screen time, or minor anxiety, and it often appears when the child is relaxed or idle. The fact that it does not occur during sleep or while speaking is reassuring and strongly suggests it is not a seizure or serious neurological problem. In most children, these tics are temporary and resolve on their own over weeks to months without treatment. No medicines are required at this stage. Most likely it is a behavioural issue. Don't give too much attention to it. You need to ignore that while he does this. Try to make a video of this event and then consult if this doesn't go away in a few days. Sometimes children do such things to get attention.
