There are many reasons. She needs consultation with a Gynaecologist in your area (preferably the same Gynaecologist who did the operation).
Please don't ask for a list of all the reasons which can cause spotting 5 years after a hysterectomy.

It can be sometimes due to granuloma formation but meet a Gynaecologist in person, get a physical examination and pap smear done along with ultrasound of abdomen and transvaginal scan
