Some white spots on back and neck

Hi, I have some some 4-5 small white spot on back and one big one on neck. Had a history of fungul infection on 2015 and 2017. That time I got a treatment and it gone. Now I am having it again, it it fungul or some thing else can you please suggest. I have attached photo of bigger one, with this 4-5 small patch is also there. Thanks
