Hey there, I understand how difficult it might be to deal with social anxiety. You have taken a step in the right direction by reaching out for help. The best thing you can do is consult a mental health professional. He/she will be able to guide you through and help find you a specific treatment plan. CBT or Cognitive behavioural therapy is a type of therapy that has been found to be extremely successful to help you manage social anxiety. Explain your concerns in detail to your therapist and she will be able to find out the type of therapy that will work out the best for you. However here are some things you can do alongside therapy to help you manage your social anxiety better: Practise mindfulness and relaxation techniques : Even practising a few minutes of therapy on a daily basis is said to have amazing results. It will help you manage your stress a lot better and also help you have better control over yourself. Find support groups for social anxiety : It always helps if you can interact with people who have similar concerns as you do. Talking to them and hearing and sharing experiences and also finding out how they deal with it might be a great way to help you find ways you can deal with it. Reach out to family and friends: Although it can be hard to discuss extremely personal things, talking to your loved ones will make you feel like you are supported. It will also create a sense of belongingness and help you through your struggles. Journal your feelings : Penning down your feelings will help you identify your triggers and help you reflect on yourself. This will also help you vent your feelings out and sometimes that alone might cheer you up and help you cope with social anxiety better. There’s always light at the end of the tunnel! Don’t give up!

Start taking selfies and make video vlogs of yourself. Speak with yourself in diaries then chat with strangers online. Take some routine review of physical health. Hello there! I understand your situation is quite grave because of it's early onset and persistence till date. Like you rightly mentioned, you are able to see the difference in the intensity when not in a social set up. Identification of specific social setting(s) and related behavioral rehearsals would help you.
Next Steps
consult a mental health professional so that a structured program/therapy can be decided and  worked upon.
