My 2-year-old son has a very small hard bump on the scrotal skin. It feels like a tiny rice grain, is painless, and has no redness, swelling, pus, or fever. We noticed it today. Weather here is very hot (35–40°C). It appears superficial on the skin and not inside the testicle. What is this? Is observation enough or does he need examination/treatment? We noticed it last night only and attached images for reference

From the photo and description, this looks more like a tiny superficial skin lesion such as a blocked gland/epidermal cyst or small follicular lesion rather than anything inside the testicle.
As it is:
• painless
• very small
• superficial
• no redness/pus/fever
simple observation is reasonable for now.
Keep the area clean and dry. Avoid squeezing or repeatedly touching it.
Please get examined if it:
• increases in size
• becomes red/painful
• develops discharge
• causes scrotal swelling
• or if you feel any lump inside the testicle.
Greetings
The images appear to show a tiny, firm, skin-colored nodule on the surface of the scrotum. This resembles conditions like milia (small keratin cysts), molluscum contagiosum (a viral infection), or a benign epidermoid cyst.Although it seems superficial, a pediatrician should examine your son to confirm the bump is not related to internal structures.Management options depend on the diagnosis and may include simple observation, topical medications
