Skin tearing. My next course of action?

So the story started while I was coming back from my village. I was in train and while urinating I noticed I've lots of smegma build up in my penis. I never saw it while I was at home (Hyderabad) as I used to do foam wash which unfortunately I forgot to take it with me while I travelling and used soap instead. So I tried removing it with my hands as I had nothing to remove and since it is 2 days journey I was more disgusted with it. After coming back in Hyderabad I was not able to retract the foreskin and then I noticed there's are so many skin tears around the forskin and today I saw blood coming out too. Right now I'm cleaning it and applying boroplus (antiseptic cream) to it. What should be my next course of action? Thank you for the advice in advance.
