Skin problem in fingers...

I have been suffering from this skin problem since long...my fingers have skin disease patches...the skin at that point itches..and also now it has increased so much that those patches are developing cuts and they pain a lot..the cuts also bleed sumtimes and it is very hard to even fold the fingers sumtimes.
