For the 39-year-old female with persistent right shoulder pain and normal X-ray, Harrison’s Manual of Medicine recommends the following approach: Initial Evaluation Since the X-ray is normal and there's no history of trauma or radiculopathy, the most likely causes include: Mechanical conditions like rotator cuff tendinitis, bursitis, adhesive capsulitis (frozen shoulder), or calcific tendinitisHARRISONS-Manual-of-Med…. Conservative Management Options NSAIDs: For inflammation and pain control. Physical therapy: Especially important in cases of tendinitis and adhesive capsulitis. Local glucocorticoid injections: Effective in bursitis and frozen shoulder. Rest and activity modification: Avoid aggravating movements like abduction or internal rotationHARRISONS-Manual-of-Med…. Condition-Specific Notes Tendinitis: Pain is dull/aching and worsens with movement; respond to NSAIDs, PT, and sometimes injections. Adhesive capsulitis: Painful restricted movement; physical therapy is the mainstay. Calcific tendinitis: Sudden severe pain; usually self-limiting but may need ultrasound-guided aspiration in resistant casesHARRISONS-Manual-of-Med…. Avoid unnecessary treatments No evidence supports surgical intervention unless conservative treatment fails over time. Avoid prolonged rest as it may worsen capsulitis. This structured approach can help resolve most cases of non-traumatic shoulder pain with normal imaging findings.
