I am a 19 year old MBBS student. I have noticed that upon full extension of my fingers and other body parts like neck joint, it starts shaking. I have noticed this while taking selfies. While clicking pictures with my hand at mid-prone position, it shakes moderately. Also, during public speaking and nervous situations, the shaking becomes more vigourous.

This happens to everyone who is in mbbs.. One tries to find faults in normal parts of the body after reading the medical books. I don't think there's any problem with any of your body part..you need to follow relaxation techniques to reduce the anxiety.
