Severe health anxiety please answer
Hi , I had anal abscess two times last year and don't have any drainage or reabscess since a year, doctors said there is no fistula track or anything.but I am not able to get over it.i keep thinking what if I reabscess what if the track silently progress into other organs,will turn to rectovaginal silently,I am not able to overcome this in my mind,it gets difficult to travel and also plan things.please help.

Please don't burden yourself with all these "ifs" like if you get abscess or fistula etc. Just focus on your diet and lifestyle accordingly. If you have any signs immediately consult a General surgeon and get evaluated further.

Your fear is , if it abscess reforms and gets progressed to other organs. It’s not how it happens without you having any symptoms of it in initial stages, understand that anal area is very sensitive part and any pathology there causes symptoms at a very early stage, symptoms could be- pain, discharge, swelling, discomfort ,diarrhoea , unexplained fever . In any of these symptoms occur you should consult a general surgeon/ proctologist .

If abscess recurs, MRI perineum in advised for visualisation of any abscess or sinus or fistula.

Having managed hundreds of fistula and perianal abscess cases and having presented rare perianal cases at clinical meets—I can tell you that a 'silent progression' into other organs or a rectovaginal fistula without any clinical symptoms (like pain, discharge, or swelling) is highly unlikely. To give you a better assessment, I have a few questions and recommendations: 1. When was your last imaging (like an MRI or Endoanal Ultrasound) performed? Was it done before or after your last incision and drainage (I&D) procedure? 2. Risk Factors & Prevention The best way to prevent a recurrence or the formation of a track is to manage the local environment and bowel habits. - Prevent Constipation: Straining can increase anal canal pressure. Ensure a high-fiber diet and adequate hydration. - Local Hygiene: Continue maintaining good perianal hygiene. - Underlying Conditions: If there is no history of Crohn’s disease or other inflammatory conditions, the risk of a simple abscess spontaneously becoming a complex multi-organ track is extremely low. 3. Tracks do not move 'silently' without causing inflammation or discharge that you would notice. Hi. First of all, you didn't have any symptoms in the last year. When there are no symptoms and there are no signs or symptoms of fistula in another, even a clinical examination will show no fistula. there is no need to get anxious just by thinking you may progress to complex fistula. You may travel without any tension now.

What you’re experiencing sounds like anxiety after a past illness. Stay observant for new pain, swelling, or discharge, but you can safely resume normal activities without fear.
