Since yesterday, I have been experiencing severe loose motions, possibly due to food poisoning. I have already had more than six episodes of diarrhea. I also had a mildly high temperature initially, which has now come down. I feel a burning or hot sensation in my stomach and have an urgent need to use the restroom each time. I started taking Metrogyl 400 mg, but I am not sure if it is helping. As I am a foreigner in India and not familiar with the local medications, I would appreciate medical guidance.

Your symptoms (severe watery/loose motions >6 times since yesterday, initial mild fever now settled, burning/hot feeling in stomach, very urgent need to pass stool) are classic for acute infectious diarrhea, most likely bacterial food poisoning (common causes in India: E. coli, Salmonella, Shigella, Campylobacter, or sometimes parasites like Giardia/Entamoeba).
Metrogyl (metronidazole 400 mg) helps against some parasites (Giardia, Entamoeba) and anaerobic bacteria, but it is not the best first-choice antibiotic for most bacterial food poisoning in India — it usually does not help against the most common pathogens (E. coli, Salmonella, Shigella). That is likely why you are not seeing clear improvement yet.
This is not mild anymore (high frequency, burning, urgency) — you are at risk of dehydration and electrolyte imbalance, especially as a foreigner (possibly less adapted to local pathogens and heat).
Next Steps
1. Go to a doctor or hospital/clinic today (do not wait another day)
• Best options in India (especially if in a city):
• Private hospital emergency / OPD (Apollo, Fortis, Max, Manipal, etc.)
• Good multi-specialty clinic or gastroenterologist
• Avoid only medical shop treatment now — you need proper examination
2. The doctor will most likely:
• Examine you (check dehydration: dry mouth, skin turgor, sunken eyes, BP, pulse)
• Give oral rehydration salts (ORS) — most important treatment
• Possibly start better antibiotic (examples below)
• Advise blood tests (electrolytes, kidney function) if dehydration is significant
• Stool test only if symptoms continue >3–5 days or blood/mucus appears
Health Tips
• Stop Metrogyl for now — it is not the right choice for most cases and can cause more stomach upset
• Most important: Drink ORS continuously
• Buy WHO-ORS packet (Electral, Enerzal, ORS-L, Prolyte, Peditral, etc.)
• Mix 1 packet in 1 liter clean water
• Sip 100–200 ml every time you pass stool + extra 200–300 ml per hour
• Goal: pass clear/pale yellow urine every 3–4 hours
• Safe medicines you can take before seeing doctor (only if already available):
• Loperamide 2 mg (Eldoper, Imodium) — 1 capsule after each loose motion (max 4–6 per day) — reduces frequency & urgency (do not take if fever >100.4°F or blood in stool)
• Paracetamol 500–650 mg every 6 hours if any fever or body ache returns
• Zinc 20 mg daily (Zincovit syrup or tablet) — helps recovery & shortens duration
• Diet right now
• Only clear fluids + ORS for next 12–24 hours
• After that: plain khichdi, curd rice, banana, boiled apple, coconut water, sago (sabudana) water
• Avoid: milk, spicy/oily food, raw vegetables, fruits with skin, tea/coffee, carbonated drinks
• Danger signs — go to hospital immediately (emergency) if any of these appear:
• Unable to drink or keep fluids down (vomiting everything)
• Very little or no urine for 6–8 hours
• Severe weakness, confusion, dry mouth/tongue, sunken eyes
• Blood in stool
• Fever >101°F returns
• Severe abdominal pain
You are doing the right thing by recognizing it early — most food-poisoning diarrhea improves in 2–5 days with proper hydration + correct medicine.
The key right now is aggressive ORS + doctor visit today — do not rely only on Metrogyl.
If you can tell me your city (so I can suggest nearby good clinics/hospitals) or if any danger sign appears, update me — I’ll guide you further.
Take care and get medical help soon
Take tab sporlac one every 8 hours for 3 days.
Take electral powder, other hydrating, salty drinks.
Visit nearby hospital for clinical evaluation, i.e. blood pressure, pulse temperature and state of hydration.
1.Hydration is the most important treatment
2.Start ORS (Oral Rehydration Salts).
3.metrogyl definitely work and start with sporolac probiotics. Most cases improve within 1–3 days with hydration and rest.if blood in stool present ,consult immediately to local hospital .
