Severe depression, I guess!
I think I have been in stress from several years but in severe depression from last 1.5 years. I don't know if it is just sadness or I'm actually surprised suffering from depression. Some of my symptoms are - attention deficit, irregular sleep, irregular diet, loss of interest, low self esteem, low confidence, forgetfulness, anxiety, all day sadness, mood swings, and stress headache.

Hello Screen yourself for clinical depression. Do a self rating test of depression like Zung's self rating depression scale which you can easily find online and if its indicative of moderate to severe depression consult a psychiatrist in person for evaluation and treatment.
